Sign up to become a Strive Compounding Pharmacy provider, and begin filling your patient's prescriptions by mail or pickup at one of our convenient pharmacy locations. After your application has been accepted by a member of our team, we will set you up with a login to LifeFile, the portal where you'll submit prescriptions for your patients.

3 Quick Steps to Start Prescribing:

  1. Fill out the "New Clinic Inquiry Form" located below.
  2. After you submit it, we will email you an additional form to collect information about specific providers at your clinic who will need to prescribe.
  3. If you choose the "Bill Office" option below, we will also include the link to a "Clinic Billing Form" in that email.
